Extension of an Original Assessment Certificate

 

The Act allows for an extension of a current assessment certificate to cover other companies intending to import and/or manufacture a new chemical. If one or more companies wish to import or manufacture the chemical, their name(s) may be added to the assessment certificate, provided that the holder of the original certificate agrees and that a certain number of other criteria are met. This includes completion of Form 14, which gives permission from the original certificate holder to add additional companies to the assessment certificate. Once the forms, fees, and sufficient data have been received, draft amended reports will be provided within 45 days.

In addition to the relevant forms and fees, an application for an extension should consist of:

• For a PLC, the data items listed on Form 1-PLC;
  OR

• For any other chemical or polymer other than PLC, paragraphs 1, 2, 3, 5, 11 and 12 of Part B of the Schedule, that is, the identity and composition of the chemical, its use and volume of use, and a label and MSDS;
  
  AND

• Supplementary information on matters affecting occupational, public and environmental exposure if the application contains any significant differences from the original application;

• Any new health and environmental effects information that is available about the new chemical; and

• Confirmation that the applicant has access to the full public assessment report for the new chemical.
