The CEC application template has been provided by NICNAS as a service to notifiers. It incorporates standard headings under which the required information may be entered, and includes guidance material for each heading; the guidance may be seen by placing the mouse cursor over the highlighted headings.

The template should be filled in electronically using Word 97 or Word 2000, or compatible programs. The completed document should be provided to NICNAS electronically by floppy disk or CD, or by email (noting that NICNAS does not have a secure electronic document submission system at this time).

The use of the template should simplify the process of applying for a CEC permit by making the required information clear and by providing the formatted document to enter information. The electronic version will be annotated by NICNAS and retained on a secure file system. Only the small amount of information indicated on the cover page of the submission template will be published.

The template should be submitted in conjunction with (the below forms can be found on the main CEC page):

• A signed copy of the required application form (Form 1 CEC),

• A checklist for CEC, a copy of Form 3 Exempt Information if exemption from publication is being claimed for any items, and;

• A copy of the Customer Agreement form signed by each applicant and each customer company involved in commercial evaluation.
